What ever you do, don't refer to them as Sir & Madam!

The formal address should be:

His Grace & Her Grace, the Duke & Duchess of.....

and the salutation should be: My Lord Duke, Madam,

How you close it is less defined. I would go with something along the lines of: I remain your most humble servant.

Within the body of the correspondence use Your Grace.

Unless of course they are Royalty. In which case they are: His Royal Highness.... and Her Royal Highness.

Etiqeutte...it's a bloody minefield!
