Well in cancer, De novo means the first instance of something so I assume if you are suffering from de novo depression it is the first time you have had it.

My guess would be a depression someone has always had, 'de nova' means from the beginning. An endogenous rather than a reactive depression which is where you become depression as a reaction to something, eg a bereavement.
