It could be ao many different things. but if the pain is so bad that she is screaming, it is serious. Go to the emergency room right now! Call the ambulance.

She needs to see a doctor, there are a number of things pain in the legs can be caused by, none of which are simple fixes. She must see a doctor. Call 911, have her seen.

It could be different things causing the pain. It could be sciatica (pressure on the nerves in her back)or poor circulation. She should see a doctor at once.

it could be a blood clot do go get an MRI or LP its either that or maybe shes on the larger side and she just cant carry her own weight anymore

Hard to tell from what details you have give. If it's that bad, get her to the hospital.
