For the same reason people judge whether or not someone is "In shape" by the shape of their body instead of their health and physical abilities? Often people say things like "she's really in shape" based on how she looks in a bikini. They have no idea whether or not she's athletic.
I think such phrases are often misused.

Like you said, PEARS AND CIRCLES, are shapes. Being fat is not a SHAPE! It is a condition that needs correcting!!!!

For that matter, Why does ANYONE want to get in shape...its isn't just fat people.

they would like to change shapes to be a more defined and better shape. Who wants to be a circle when u can be a chizzled hexagon .
