You should look up information on sedation dentistry. For someone that doesn't like dentists that is the best direction to go.

A lot of people are afraid to go to the doctor period. However point out to him that could end up having lots of problems with his teeth if he does not go. Ironically enough orthodontists are usually the ones whom don't hurt you. You might want to point that fact out to him. Had braces and the braces are what hurt when they were adjusted. If they need to pull extra teeth they will more than likely put him to sleep. I hope this helps.

Well, no matter how scared he may be of doctors, people are more scared of public scorn or death. I would show him pictures on the Internet of people that have horrible mouth diseases and/or let him know that friends and neighbors might ridicule him if they notice his bad hygiene. Consult your parents before taking any action. They are the best ones to push home the point.

Your parents should be able to force him to go. If they don't make him go to the orthodontist, what else are they not making him do? Homework? Chores? Sorry, I didn't mean to pass judgement, I don't know you/your parents, but it really is in his best interest to go. He could have self-esteem issues b/c of his teeth in the future.
