I would keep it simple but sensitive to your boss's situation:
(Boss's name),
My condolences to you and your family during this difficult time.
My thoughts are with you,
(your name)

I assume he/she lost a spouse. So I would write
"The condolences of me and my family to you and your family in this trying and difficult time. Remember this the pain of losing (enter name) is our pain. (His) (Her)Their pain is over as they are in the God's loving arms."
If it is a the loss of a child.
"I know mere words can not end your pain. Parents are supposed to out live thier children. All I can offer you in this trying and difficult time is kind words and comfort. Remember this the pain of losing (enter name) is our pain. (His) (Her)Their pain is over as they are in the God's loving arms."

Something simple, like, "My thoughts are with you during this difficult time. Please let me know how I can help."
I know your boss will appreciate your card.
