Can he hold a pencil or pen in an orthodox way? If he can’t, then don’t teach him yet. It will only teach him bad writing habits.
If he cannot properly hold a pencil, help him gain some hand strength. To help him get some hand strength, have him use toys such as Lego's, play dough, or do things like scrub potatoes or windows with a little brush. He can also trace him name in a dish or cornmeal or sand (limits frustration).
If he is ready, make sure you have him learn to write him name starting off with an upper-case first letter, followed by lower-case letters. He will be expected to know it this way in kindergarten. You can write his name several times with a yellow marker for him to trace. You can also write him name on a piece of cardboard and have him use tracing paper to trace it. Remember to keep it fun!
It's more important that he can recite his first and last name and recognize his name. If he cannot, work on that first. Here's a link to some kindergarten readiness skills. Good luck!

What worked for me is writing out the letters with broken lines and let the child trace them. My daughter was writing her name at 3 this way.

I am raising my 6 yr old grand daughter and she could write her name before she started kindergarten.
What I did was sit her down at her little table and chairs and gave her pencils, crayons, paper, etc. She has always loved to draw and stuff.
I sat down with her and held her hand gently and helped her make the letters in her name. We practiced off and on. I did not sit and make her do it, we did it when she wanted to.
We made a game of it also by finding her name in different books and when she found her name in the book, she wrote it on a piece of paper in a notebook.
Another game is to........when he draws you a picture, praise him on his drawing and ask him to autograph it for you to make it special. And when you draw pics for him, show him that you are autographing the pics you drew for him so they will be special.
When Haley started school she could write her first and last name. I was very proud of her and praised her alot when she would write her name. She was the only child in her class that could write both her first and last name.

Our children each developed differently. They learned to write letters and how to recognize their name seperately. Then they put the two together. They are 7,5,4 (2 girls, 1 boy) each can write their own name. I'd try seperating the activities and see if he can put it together. We keep alphabet letters on the fridge and they began organizing words. The first thing each did was their name.

Get him some magnetic letters that go on the fridge and let him see the letters in his name. Then have him practice writing them on a chalk board and then on paper.

There is a story about a little girl who learnt to write her name by finding the lettters. It is a fun way for your child to learn to write his name through playing and listening to the story and doing the activity. I taught my child to write her name through this fun process. Listening to a story is always a great way to learn any lesson.

my son was four when he started writing his name by himself i would put his name on paper but i would write it using dots and he would trace them after a couple of days he would do it on his own that was the best way.

try making a poem using the letter of his name. draw pictures on it and let him help color it then frame it and put it somewhere he'll look at it all the time.
repetition, magnets, blocks, and rewards can also help.
one thing I did with my oldest is put a prize (candy) in a box then wrote her name on it. I then mixed it up with other boxes and had her find the one with her name on it. helped with the recognition of it anyway.
