make sure he/she gets a lot of exercise during the day
make your home really structured with set chores, a set bedtime, and predetermined punishments for bad behavior (as well as predetermined rewards for good behavior)
i suggest keeping a posterboard on the wall and putting a sticker on it when he does something good (ie: listening when you tell him something, doing his chores, etc.) if he gets X amount of stickers in a week he can get a small toy or go to chucky cheese or something.
use time out and taking away toys/TV for punishment

I'm afraid you're a little too late. You should have started when the kid was 2. They don't call that age "terrible two's" for nothing. That is the time when kids start testing parents' limits. If you give in then, as time goes by it gets worse.
I might suggest to put your son/daughter in an outdoors activity like a sport, to channel that hyper activity.

Structure, structure, structure. And be consisent! You've got to improve your parenting skills; no means no, there are consequences enforced regardless of how many times you return them to the corner/time out, etc. Set times (with in reason) for consistent bed/nap/eating/activity time etc. And almost as important is diet. What are you feeding the child? Watch sugars but just as important - watch carbs - which break down into sugar (milk, white bread, juices, peanut butter, crackers, etc.) While most parents aren't feeding kids endless glasses of kool-aid or Mt. Dew - they are milk - which breaks down into sugar which equals hyper! At 4 it's going to be very difficult to reign your child in - but if you don't now - it'll be the law doing it for you in the teens.............. Believe me, kids not only need structure but want it. Keep them busy!!!! I have one also. Give them "responsibilities". I let my son go bazerk in his room playing with his toys, which he makes a complete mess of! Then he spends half a day cleaning. It takes that long cause he starts playing. Take them to a park to burn off energy. Right after the park it's nap time for my son. Oh, and don't give an sugar to add to the hyperness!!!!!

consistency and a schedule. I have twins that are three and I bring them to the park everyday and let them run off there energy and then we come home and eat lunch they go down for a nap. When they get up we read,color, play with there toys and then after supper they go outside ride there bikes and play and then we have quiet time read somee books and take baths and get them down to bed. I always try to keep som routine even when we are visiting elsewhere.

MIne is ADD. Yours could be ADHD. There's absolutely no reason for you to allow a doctor to medicate at this age, it'll be tough enough in the future, put it off as long as possible.
The medicine used to treat both is speed. Yes, the illegal drug. It has the opposite effect on ADD and ADHD patients. Another useful drug for this is caffeine. Honestly, when my daughter runs out of pills, I give her a Mountain Dew, and she's fine for two to three hours. Not as long as the medicine, but good enough for a while.

there is a parenting/discipline course called 1-2-3 Magic. it's designed to help parents set limits. many community mental health agencies offer this course or courses like it for a minimal cost.

kids need boundries. set the rules for behavior. he is old enough to understand hat acting up will get him time outs. If he will not get the message from a time out, remove his favorite toy for 1-2 days.
