Could be because of anyone of the following factors or a combination of them.
1. U are not getting the lenght of yr swing right.
2. U are looking up before u are hitting the ball.
3. U might not be keeping still during the downswing.

Get someone experienced to look at u while u practise at the driving range. He will be able to help u.

Keep practising, take yr time to pause between strokes at the driving range. U can try with a short backlift initially.

just a guess - good chance that you are raising your head and shoulders on the backswing and then overcompensating for it on the front swing. It could also be a problem with where you are addressing the ball - too far back in your stace for example. Get a video camera and film yourself at the driving range. Watch the tape in slow motion. That can be quite revealing.

1. You are not shifting your weight and hanging back on your back foot/leg.

2. You swinging too steep into the ball. Try to keep you left arm (for right handers) straight.

you are turning your wrists over too soon, take your club and put it right behind the ball where you want to hit it, remember how that position feels and focus on hitting that ball in that same 'feel'

you are probably standing too close to the ball. might need to back up a little and bend your knees slightly. your alignment with you feet and the ball may be incorrect also.

You are not maintaining your spine angle and are probably moving your upper body to the right on your backswing.

are you keeping your shoulder straight in the fellow thought keep head down left arm straight
