Inmates are allowed to self-declare their religion and it is kept in their file so that religious observances and dietary restrictions can be honored.

In the United States yes the prison is required to keep a record of what the inmates religion is. However, it varies greatly form state to state and even from prison to prison how much if any accommodation is given to the inmate for his or her religious practices. States are sued by inmates all the time for religious persecution and denial of religious freedoms Almost all prison chaplains are some form of Christian, a few are Islamic or Jewish. If you are not in a "main stream religion" it is very difficult go practice your faith, with out being harassed.

All prisons are required to ask New inmates there religious preference and they even cater to certain religious requirements (like kosher meals and even smoke houses for Native American indians) although not ALL (like no peyote)

Yes they do along with other sociological information. However, the inmate does have the right not to disclose their belief system.

I think it probably on your personnel records, but I don't think it is meant to be used for anything, except I am sure they can go to church in prison. If I were in prison, it would not matter to me. I am a baptized, church going, Southern Baptist.

I don't know for sure, but I would guess yes. Some individuals may need to pray at a certain time and they may allow that.
