Birth control isn't 100% so there is still a small, yet existing, chance that something can happen.

Sperm also isn't just released during ejaculation either. Some sperm cells can be released during intercourse, before ejaculation.

The only sure birth control is abstinence. Generally, one form of birth control is all that is needed. My wife and I successfully used condoms to space our children. My daughter used two forms of birth control, due to health issues, and got pregnant anyway. Fortunately, mother and baby are okay. So the short answer is if you think you are pregnant, then you might be.
