Okay, take a look at these answers.
He wants to kiss you, buy you a drink and hook up.
This could be liking you for you or it could be that he
wants to have sex with you but not be involved.
Use the survey as a springboard to ask him out on
a date. Take it slow and see where his real intentions
are.
If you do go out, make it a double with a friend and their
date. That way it's not awkward and more importantly
you're with someone you know and trust that can watch
out for you.
In this day and age, a drink can be spiked easily.

It sound like you guys have not known each other very long? Maybe he is still trying to figure it out, also from his answers to this survey it looks like he is not just trying to jump in bed with you so that should be a good thing hard to find guys like that now days. Give it awhile just hang out and talk learn about what he likes and give him time. He might not know if he likes you yet you know when he figures it out himself. sometimes it's not an over night thing.
