It seems your habit to be corrected. Try "AIR" method, Attention, Intention, Repetition, while doing something. It quite well works on you.
You cannot concentrate longer than 10 minute, because you are boring of what you've done. Attention is needed when you meet something new. It will make something to be familiar with you. Intention is needed when you are begin to involve something in you. Repetition is needed for you to make it more familiar to what you've done. Try this step by step.
If you take this method, I'm sure you can remember much.

I have the same problem I get my conversations with my friends mixed up. I can't seem to concentrate on something more than 30 seconds. Maybe you have attention deficit disorder or either you smoke w**d!

Try as much as possible, try trainning yourself. Stop using alarms to wake you up, for example, and try remembering what you want to do without previous notes you hang (on the wall, fridge...) for yourself in the morning. Good training brings good minds. Other than that, I recommend a doctor, I had a friend like you and turns out he was short on the vitamin B12!

Just do a simple meditation excercise like counting from 1 to 10 and backwards. Try to keep ur mind cool. dont think much of other's problems.
