One thing I have noticed in the past is that when using Yahoo mail and others, the dialogue box to upload the attachment can come up behind other windows already open on the screen.
Therefore you don't see it and think nothing has happened.
Always check the taskbar to see if another window has opened and try just minimising other windows to double check.

how long do you wait? what are you useing to send and attach emails?

if it's something like hotmail, or yahoo mail you will have to wait a while after hitting the attach button, this is the file being "sent" which can take a few seconds (for a small file) to a few minutes (a big file) heh go watch tv for a few minutes and check on it.

if you useing internet explorer if you see a small blue bar growing after you hit it, it's just being sent, just wait and don't press the button a million times, that will just start it over and you'll have to wait longer.

Do you use a normal browser-email (i.e. Yahoo/Hotmail) or is your email accessed by Outlook? If so, it could be that Outlook has problems connecting to your email server. To check this, you can try attaching your files to an email on the internet (on your mail servers site).
