Walk up to her and say, "Hi, I'm _______. Would you like to dance?"
I mean what can she do? Say "no" and then you just go on your way and ask someone else.
If you have been crushing on her, ask a couple of other random girls first, and then ask her. This way, you have a couple 'under your belt' before you get to her. (You know, to work up your nerve!)
Seriously, girls like it when guys they've not met before ask them to dance. The girl is just as nervous as you are. So think of something to talk about while you are dancing if it is a slow dance.

2 words- wanna dance. i know that sounds too easy but it is the only way to do it. yes they might say no but they could say yes.

just ask her she can only say yes or no but set yourself up for a no that way if she says no it wont hurt so bad...
