Remember (verb): To recall from your own recollections. EXAMPLE: You read the chapter in History and studied for the test. You remember what you read last night when you read the questions on the test.

Remind (verb): To assist recalling information for yourself or someone else. Some people will write themselves notes on the refrigerator with dates and times of appointments to remind them of an appointment. They may have not remembered the appointment without a reminder.

If your mother forgets (or doesn't remember) your birthday, then you have to remind her by writing your name in the box on the calender for the month and date of you birthday.

I would look at the two definitions in a dictionary to get a good idea on when to use which word.
