think of what he likes and get him something related that he would never expect or couldn't get himself.
For my friend's 30th birthday I got him a baseball signed by the whole '99 mets team. It took some doing ,but it is exotic and a treasure he will never forget or sell.

a big badge saying your an old giffer and i am not! buy him something nice like choccys a small gift! then send him on a trip somewhere, golf,theme park it really depends on how old your brother feels! buy him a card with a clothes voucher in it! xxxktxxx

Check giger.com for some unusual jewellery etc. He's the guy who did the artwork and alien design for the first Alien movie. It's not cheap though.

Give him something he would never splurge on for himself. My brother likes Hallmark ornaments...so I gave him a gift cert. to Hallmark. If he likes to golf...a round of golf?
If he enjoys baseball...a pair of tickets to his favorite team? If he enjoys cars....tickets to a car race or cert. to an auto-parts store?

i can't suggest anything, you know your brother more than we do. Get something that you think your brother needs or something that relates to his hobby, in this case you wont miss, he will like it.
