You don't need a mouth to scream. You can scream without being heard. The silent scream is identified as being one that expresses such levels of profound grief that one may scream, but not make a sound.
I believe Al Pacino potrayed this type of scream in Godfather III, when his daughter was killed on the steps of the Opera house.
