It all depends on dosage an length of therapy. It is metabolized in the liver

This depends on how much your take. Like any drug or substance, the dose makes the poison.

Tylenol PM has two ingredients: acetaminophen(APAP) and diphenhydramine. The APAP is the same pain reliever that is in regular Tylenol. As long as you don't exceed a total daily dosage of 4000mg (4 gm) then this will not affect you long-term. More than 4000mg/day can lead to liver failure. Diphenhydramine is the same antihistamine used in Benadryl. Benadryl causes a lot of drowsiness and so the makers of Tylenol PM use this product just for its ability to induce drowsiness instead of allergy control. At very high doses this medication can cause hallicunations and difficulty with salivation, lacrimation, urination and defacation.

Like any medication used to induce sleep, it does carry a risk for dependency so use it only as needed and not as a long-term solution. However, if you take this product as directed you shouldn't see any long-term complications arising exclusively from its use.

Tylenol is tough on your liver. An overdose of Tylenol can shut it down completely. It is especially dangerous when you'd been drinking.
