iam out side the kingdom for vacation &my visa finished

Contact your local saudi consulate or embassy,
they will ask certain papers from you and your sponsor in KSA, and a written request from your sponsor to the consulate/embassy.
Don't worry it can be done but if you have valid reasons,
the best valid reason for expiring the visa is medical reason.
