Bite-wise: Hornets, Horse flies(worst), Deer Flies (nasty experience),

Poisonous bites that are very painful:
Scorpion (any kind), but some African Scorpions are deadly. (these are stings, not bites) Biters include Centipede of South America...very painful, very dangerous, Brown Recluse Spider (wrecks havoc with tissue), any poisonous spider (lots of them).

Does "death" qualify as "painful"??

Some big horseflies have a bite like a big needle going in. OW! But if you're talking venom, I've no idea.

Large ground beetles will tend to go on the attack, using their strong mandibles to forcibly persuade a predator to seek out easier prey.

Ironically, the mandibles on a rhino beetle (the strongest insect) don't take the price because they don't attack.

I've only experienced mosquitos, ticks, and spiders but the spider was the worst by far...it was a black widow!
