Human error is probably the biggest reason. Some airport codes are so close and just off by 1 letter that the person loading the luggage mistakenly put it on another flight.

The person at the ticket counter could also have inputted the wrong destination airport code. That is why they suggest you check the luggage tag to verify the correct airport code before the ticket counter agent takes your baggage and puts it on the conveyor belt.

Maybe your flight had too many checked-in pieces of luggage and they had to put your luggage on another flight and somehow it was mistakenly flown to another city.

I just hope your luggage doesn't end up in a better city than the one you are flying to.

Technology isn't perfect, but it can also be put down to timing (missing loading the luggage), misprints on the label so it goes on the wrong flight or your basic human error when the baggage handlers get it wrong, drop luggage during transportation to the 'plane and don't notice, etc.

Human error. Bags placed on wrong carts going to another aircraft.
I have only experienced my baggage left behind at point of departure.

Name tag, gets torn off, or mis read, ever see who is doing the baggage handling, most don't have a clue where any place is.

They can get lost for various reasons. Such as misplaced on to another flight, lost on the airport conveyor belt (see US Airways at PHL) and of course it could have been stolen.
