In the cold war era, peace was guaranteed by the MAD outcome of a nuclear war between the USA and the Soviet Union. MAD standing for Mutual Assured Destruction. Nowadays, with small powers having small lots of nuclear weapons, there is a possibility of a limited nuclear war which would not have so disastrous ecological consequences, not more than the airborne nuclear tests that were performed in the 1950s. The consequences to human life would be horrible, of course. It would also generate such hate, for generations between the countries at war that it would not be politically «profitable». The reason for such hate is that most people see the nuclear weapon as the Devil himself.

We should, if necessary.
But no big war has been fought yet, after World War 2, so no nukes used yet.
I dont think any of the nuclear power will hesitate to use it, if they find themself into a losing war.

It's because of the Cold War. A-bombs are extremely destructive. If any government launches such bomb, the enemy will do the same, and it ends up in total extinction.
It also has far-reaching consequences. The area becomes unfertile and highly dangerous for humans for decades. See the health situation in Chernobil or in the bombed Japanese cities for reference.
