the INS office where his status was adjusted? Cause he doesn't know himself. And he doesn't know what city he applied in either cause he was too young.

Its the application to replace permanent resident card....and it says Form I-90 on the bottom.

Yes, the I-90 does ask those questions.

If your husband can't ask his parents, I suggest that he fill those blocks out as best he can: he knows his parents' names obviously, and where he was born. Since they adjusted status rather than entering with immigrant visas, and he doesn't know where or when that happened, he should write in those three blocks "I was an infant and I don't know."
