I believe the space probe Voyager is the fastest since it had to have enough speed to leave the solar system.

In particle accelerators pieces of atoms are made to go nearly the speed of light. (like 99.9995% or something). Particles with short lifetimes have been made to go so fast that their apparent lifetime is million times longer than their lifetime at rest.. that takes significant time dialation.
There have been wind tunnels made that go to where an underground nuclear explosion is about to happen, and objects in those tunnels have been tested at mach 200. They are destroyed by the blast, but the nuclear explosion send the gasses past at very very high speeds.
There are high velocity gas guns that shoot aerodynamic testing projectiles at higher mach numbers.. but those are useful only up to about mach 6 or 7.
The space-shuttle and several rockets have gone very fast (mach 15-25.
There are rumors that the soviets have experimental vehicles (unmanned) that top mach 16 in the atmosphere without having to fall from 600,000 feet to do it.. they do it based on thrust not on potential energy.
The environment is significant qualifier for fastest... going fast in the air isnt nearly as hard as going fast underwater.

Actually the fastest man-made craft has been Deep Space One... It reached speeds of about 10.2 km/s, far faster than the shuttle or voyager, using a xenon ion engine...
