I would really like to say her personality, but thats pretty hard to look at as a 'First Thing', so I would have to go with The Eyes, there is nothing like a girl with lovely big bright eyes, no matter the colour!!

Her personality, if that's good she will have a wonderful smile that shows she is for real.There are allot of stuck up women out there that have pretty smiles but there personality suck.

Honestly, all guys if they are not hypocrites and abnormal look at their bust/figure, then her face and other things...but definitely the first thing is her bust...
