mainly a combination of all 3. For instance, i have a friend, (he's 19) who eats a ton of candy and other food with pretty low nutritional values, as well as not working out. somehow, he only weighs about 120 (he's about 5'10") and has very little fat on his body. That would be attributed to his metabolism. of course, as you age, metabolism slows, but if you lead an active lifestyle, you can help counter that.

mostly it is imbalanced diet & living style ( less physical activity )are the main causes for gaining unwanted weight.
