Here's one we used to get a kick out of. Drive him a mile from either your house or his or whatever. have him get out of the car and strip completely naked. he will put his clothes in the car with you and you leave. drive back to the house and hang out with your friends. The deal is he has to make it one mile completely naked back to the house. This is much harder than one would think and can sometimes take quite a few hours. Especially when you have to somehow get across an intersection. With the risk of being caught and going to jail, etc. it is quite a tough dare.

Have him go to work/school last day before long vacation wearing ordinary clothes, but a huge codpiece of gauze bandages. (He can include a plastic tube like a catheter and a drip bag, if he wants.) And all he's allowed to tell people is that he was 'injured'.
