I don't think spanking or pulling hair is out of line. I think it's only courteous to ask before handcuffing or blindfolding someone...I doubt they'll get a 'no,' but it's nice to know beforehand that you'll be restrained or binded. A guy in a dress....yeh that's far out. A costume would be a great surprise though! I'd love to walk in and find my man in nothing but a toolbelt, or a fireman's costume...

As far as I'm considered, anything non-consensual is "way out there". Of course, your question isn't asking that, but what should be discussed beforehand versus what is sprung as the last minute.
I think that it is important for couples to have a general set of ground rules that encompass what would be fair to have as a surprise and what needs to be discussed beforehand. I suppose for me personally, anything that involves other people should be brought up beforehand as it is adding an additional level of consent. Basically, when you have "threesomes" or the like, the number of interactions increase, so that needs to have some level of participation.
For just about anything else, having it as a surprise shouldn't be a problem as long as the opportunity to refuse is an option. Otherwise, it would just create resentment. But it is a pleasant gesture of thought to offer one's partner a new option as if it were a present.
