Improper storage or a bad cork. The vinegar taste comes from Oxidization which is a direct result of air getting into the wine after it has been sealed.

Acetobacter infection. This bacteria is also known as Mother of Vinegar. It can be introduced intentionally or by accident during fermentation in an open vessel.
A closed bottle of wine will not spontaneously turn to vinegar.
Just as yeast causes fermentation and converts sugars to alcohol so acetobacter consumes the alcohol and converts it to acetic acid - vinegar!

The problems most common with homemade wine batches are wild yeasts and acetic bacteria. Acetic bacteria converts alcohol into acetic acid and could turn your wine to vinegar.
