Define "fast food" in India. For example, Indian Coffee House is a popular place here in Kerala, and it's the only place where my husband will take me, because of clean facilities and consistent and wholesome foods. "Hotels" seem to be less reliable in cleanliness and food content

1-- cleanliness of kitchen/employees Indian Coffee House employees are part owners, so they have a pride in their presentation. This is a big difference from the small hotel owners.
2-- food resources-- a legitimate chain probably buys the legal meat from the gov't butcher market. Another point to bring up, is where do they purchase the meat? Is it from a government butcher or from the illegal place around the corner.

Remember that pesticides and pre-processed foods play a big role in reducing the nutritive value in food. You might try a few different searches on Wikipedia for definitions of "nutritive value" and fast food...

I'd feel sad if 64% of the Indian population turned obese like in my country (USA) because of fast food. A good documentary regarding the results of eating American fast food is called "Super Size Me."
