In 17 years as a Vet Tech, I have NEVER heard of any vet doing a neuter on a cat through the abdomen. If there were undecended testicles, they would open the abdomen, but a standard neuter would NOT be done through the abdomen. It is ALWAYS done through the scrotum!!!!!!
Feel for testicles or have the vet tell you. Males are pretty easy.
If there is a penis and no testes, it is MOST LIKELY a neutered male. Undecended testicles on BOTH sides is pretty uncommon.

Yeah...LOTS of neutered male cats spray, so you can't go by that!!!! There are many females that spray too!!!!!

If you or the vet are still not sure (in a case of undesended testicles) have a testosterone level run. You are sure it is a male??!!

No nuts, but also if he was fixed then he wouldn't spray. I know it sounds silly but male cats, to mark their "territory" would spray everywhere and it would make and awful smell...but if he sprays he's not fixed.

I know it sounds gross,but the sure way is to feel his scrotum. If he still has testicles it will be obvious. However,be cautious. Kitty might object!
