This pillow form is shaped like a circle cut in half. It is about 15 inches long. I have no idea how to cover it with material and how to do the ends and make it look good. I tried to cut the ends separately and attached them, but it looks bad.

I assume you mean it is 1/2 of a globe. cut 2 circles...one the size of the flat side, but don't forget to add about a inch extra around it for the seam. The other piece should cover the rounded 'bump' side and also leave about 2" extra for the seam.
Now sew a long stitch around edge of larger piece...and pull thread to gather. fit it over pillow to see size you need it to be, When you have that...then secure thread so your gathering stays put. Then sew both pieces together leaving an opening (to insert the pillow.)Turn inside out, and you have your cover...insert pillow thru opening and hand stitch closed or apply velcro or other fastener.

Set the form on the end with the semi circle facing upward ontop of your fabric...now bring the ends up toward the top and cut the fabric so it is in a semi circle with 2 inches of overlap.
Fold the overlap down on each side so your raw edges are not showing and blind stich the
cover across the semi circle.
