"Maru.." and "fashionista" probably have it right. Depending on who you talk to, even in Spain, the two "vulgar" words in the phrase mean different things.
"Coño" in the past referred to female genitalia or the clitoris in particular. (Probably from the Latin "cunnus".) Very few cultures still keep that reference (e.g., Cuba does), and it has the same vulgar "strength" as "shi*t."
"Puñeta" was always identified with the act of male masturbation (from "puño" which means "fist.")
