See a dermatologist. This may be caused from nutritional or hormonal imbalance. The doctor will be able to find out exactly what is going on.
In the mean time, try to eat right (lots of fruit and veggies, lean protein, whole grains, and low fat dairy products), drink lots of water, and get lots of sleep. Consider taking a daily multi-vitamin as well. These measures will ensure you are getting enough nutrients from your diet, and taking care of your body.

The grey hairs are genetics, you can't do anything about them but don't pull them out, it just makes them worse.
For your hairloss, If you are a girl you should be taking Ferous Pills .Ask any pharmasict for those pills. They are like vitamins but are very important for women, as women lose blood cells every month and along with that they lose the ferous for making more blood cells that eventually it will become Talasemy. Hair loss is just a warning sign. And if you are a boy try taking vitamins and make sure those vitamins have "MINERALS" in them.

Its normal for many people. I went to high school with a couple of guys who went through teh same thing, early hair loss and gray hair. You can talk to your dr. about it and see if he/she may have any suggestions.

As far as prevention, I can't really help you. But have you tried visiting your doctor or a clinic? You should really get that checked out to see what's going on.
