I think you are asking about a Malted Milkshake and a Milkshake without the Malt added?
Actually, the malted milk shake dates from the 1950's and it is quite a tasty treat. In those days, there really were places called Malt Shops.

Malt powder is a light beige colored powder made from extracts of malted barley, and is actually a little bitter in taste if eaten straight out of the can. When mixed into a milk shake, it takes on a whole new and wonderful flavor!

If you have tasted Malts at DQ there is a world of difference than at a regular ice cream parlor. Malts at DQ are made with ice milk not ice cream. Malt is malted milk and is a powder that is used by all the malt makers across America. Shakes only contain the flavor that one picks plus the ice cream and milk. Both are blended until it is a nice consistency.

A malt has an extra ingredient a type of granulated substance called malt which is added to the ice cream, and and milk, than blended. It gives it a good taste, which I am very partial to. It is similar to regular Ovaltine, but not quite.
A milk shake is simply milk and ice cream blended, which you can add different flavors to like chocolate syrup, or straw berry syrup, etc.
You can also add different flavors to a malted as well.

A shake is just your ice cream, milk, and flavorings. A malt is those things plus malt powder. You can find it in the grocery store too.

A malt has malted milk in it. Malted milk is a powder made from milk, sugar and malted barley. (It's what's in the middle of a Whopper!)
Milk shakes don't have malt powder in them.

Umm I think malt's are a bit thicker and have little malt balls in it I think...I don't know, but I like malts yum yum.
