Yes, it's not 100% effective. My friends wife got pregnant with fraternal twins after her tubal. That means not only 1 , but 2 eggs got through.

Not likely that the ends would come loose. What can happen sometimes is that the tubes reconnect.

When I had mine done my doctor said about 1% of tubal as well as vasectomies fail.

even if you were cauterized, it could happen. i know a mom who had a permanent ligation who conceived again. i myself didn't, but it can happen
