Yes, it's not 100% effective. My friends wife got pregnant with fraternal twins after her tubal. That means not only 1 , but 2 eggs got through.

Not likely that the ends would come loose. What can happen sometimes is that the tubes reconnect.
Kinda like when two trees grow next to each other, after a while instead of two trees, you have one common tree(hybrid).
Body tissue will always try to reconnect when they touch. The Dr. should have made an effort to prevent this, but sometimes it happens anyway. Rare, but it can happen.

When I had mine done my doctor said about 1% of tubal as well as vasectomies fail.

even if you were cauterized, it could happen. i know a mom who had a permanent ligation who conceived again. i myself didn't, but it can happen
