Because you were probably wet at the time and the sunblock didn't stick well to your skin. Make sure you're completely dry before applying sunscreen on again. That mistake happened to me twice.
Put some aloe lotion on the affected area and it should feel better.

Because you exposed your boobs to the sun. Next time, try sitting in the shade, and use a high SPF, such as SPF45. The best ones are waterproof.
I can tell you how to help the peeling, at least next time. It might be too late for this go round.
Next time you get a bad sunburn, take some really cheap hair creme rinse, like Suave (cheap, but good). Get in the shower and wet the burned area. Step out of the water and slather the burned area with the creme rinse. It will burn like the very devil, but keep it on for five or six minutes before you rinse it off. The red will turn brown and if you kept it on like I said, you shouldn't peel either. Best wishes.

Well, you get burned worst on the parts of your body that are directly facing the sun. This is why shoulders and noses and the like are at risk when you're walking around in the sun. They stick out more than the rest of you. I would imagine that this also applies in this case...
As a guy, I don't have your particular risk for burning on the chest, but I still gave up on running around in the sun without a shirt on. Even when I'm swimming, if it's sunny out, I usually keep some kind of dry-weave shirt on me. It's just too much of a pain re-applying sunblock all of the time. A lot of times, I'll have a hat on as well.

i'd agree about the boobs. I were given badly burnt very last 365 days even as on holiday in Aruba and it became my shoulders even as quite stricken me because, i could not placed on a bra (luckily, I had the fashion you should take the straps off.) and had to have my swimming gown off the shoulder.
