Once you have finished your meal, you are to place your knife and fork across your plate to signal the waiter,

Just simply tell the waiter your done when he/she passes by. If the waiter doesn't come by simply ply with your fork in your hand. And litly tap it on your plate to make a slite but noticiable noise.

It's really rude in most areas to put the napkin on the plate, unless it's a paper napkin. And if you're eating in a place with a paper napkin, don't expect much from the waiter to begin with.
Just stop eating, put your flatware on the plate or to the side of it, sit back and push the plate slightly away from you.

If you enjoy the meal, complement the waiter and/or give him a 20% tip. If you hate the meal, curse the cook and vow never to step foot in that grease trap ever again.
*Oh, for the fork. Take it, and shove it far up your ***...*

I think the official way is to cross your knife and fork on the plate. I do that and it seems to work.
