I know it's happened before, but the only incident that comes to my mind right away is a cruiserweight title fight back in 1985. Lee Roy Murphy was defending against Chisanda Mutti, and both landed right crosses at the same time in the 12th round. Both went down, and the count began for both. Only Murphy got up in time, and Mutti was counted out, so Murphy won by knockout.

Many many moons ago, as shown on a Jack Johnson era frame by frame recording......two fighters went down, and, neither beat the count. THe ref counted them both out, however, both of the fighters were exhausted, because it was nothing for fighters to fight 'til the end, that could be 10, 15, or 20 rounds. If my memory serves me right, it was very hot, the fighters went well past 15 rounds, which is unheard of today, and, the fighter who knocked the fighter down collapsed. It wasn't truly a double-KO. IN THE mid 80's there was a fight in which two fighters connected simultaneously to one another's jaw and floored each other. This wasn't a double KO because one fighter by the name of Lee Roy beat the count. THERE have been several similar incidents such as this before. You can write/ submit a request/ question to Larry Merchant, or, Harold Lederman, both of HBO Boxing to get more specifics on this question.
