go to the DMV website for your state and you should be able to download the application. You then have to have the application signed by a medical doctor stating the condition and that you need one. Then DMV will issue you the placard!!

Go to the revenue office [where you get your tags] and pick up the forms. Take them to your doctor and if you have a qualified condition he will fill them out, and you can then take them back to the revenue office and get that card.
