I do not know but it looks like he was highly influenced by Frank Frazetta who did the record covers Molly Hatchet. Several fantasy artists were influenced by him including Timothy Gerritsen & Tim Gerritson that do the graphics for video games.
And maybe Ernest Gary Gygax of Dungeons & Dragons fame.

The artists name is Ken Kelly. He was born in 1946 in New London, connecticut and after showing his art work to Frazetta who encouraged him to pursue a career in commercial art he studied under Frazetta. Besides designing bookcovers he also designs album covers. He designed two album covers for KISS and was rewarded with gold albums. Here is his website featuring his paintings and biography..

Have no idea, you gonna have to configure it out by reading his signature. But they are nice drawing.
Go on word and find those type of font, it might take you for ever, but it is worth trying.
