Besides all the other great answers you get out there you can also try this:
Excuse yourself by mentioning that you need to use the toilet, after having going into the bathroom, come out and say, "Well, since your toilet doesn't flush, I will wait, I wouldn't want to cause any more problems." That should cause the person to go check the toilet (thinking that one of the kids used it and didn't flush). Upon looking at the toilet the person will realize that the comment was made because it wasn't clean. = )

The same as you would no matter what the toilet was like. At a friends home the toilet had mold and I mentioned it to him in a nice way. I noticed the next time I used it on another day that it was clean. Sometimes I think people don't see things anymore or they don't really look at it and think oh, that is dirty, better clean it. Another time my sisters toilet had mold and it looked bad. It was partly looking bad because it was an old toilet but in this situation I did not say anything to her about it. I did talk about it with my other sister though.

Bring a small pail with some sani-wipes, rubber gloves, and a can of Lysol next time you go for a visit. Pull this out when you have to use their restroom and make sure they see you carrying it in.
You can also try running out of their bathroom screaming, while brushing yourself off and crying "the horror...THE HORROR!!"
Just had another idea...melt some chocolate in spots on the seat and leave clumps of toilet paper stuck to them. It's a pretty good hint.

give them a gift...send them some bleach and some toilet cleaning supplies and tell them to clean their nasty bathrooms!

you shouldn't treat them differently because of that, but I would definitely tell them to clean that up because it is disgusting.
