I think you're supposed to change your toothbrushes and mascara every 3 months or so. So, go get an oil change, get a new toothbrush and makeup, and count down til the next 3 months! (or 3,000 miles)

There's no set time. The answers about changing after an illness are all good. If you have not been sick, you should change your toothbrush when it no longer looks like a new brush. One of my patients said, "I get a new brush when my old one starts to get a little bristle sprung." Not a bad description. If you want to be really neurotic about it, buy two brushes the next time you get a new one. Every now and then, compare your "in use" brush with the brand new one. When you can tell the difference, it's time to start using the new one and buy another one to compare to the "in use" brush the same way. Of course, once you've done this once or twice, you wont' need to keep one in reserve.

If you use only 1 toothbrush change every few weeks. An alternative is to keep about a dozen toothbrushes in rotation, using each for a day.

I haven't changed it in about 9 months. But I put it into just boiled water every 2 months or so and the bristles aren't worn out. It works great!! I let it sit in the boiled water for like 10 minutes.
