When I was about 8, I was riding my Shetland Pony and the saddle we had on him was a little too big. Borrowed it from a friend with a larger size pony. Thought we had it fixed enough. Well, the pony got spooked by a bear in the woods, I was riding along side my parents on their horses, and he just took off. I couldn't get him to stop. He made a 90* turn and the saddle slipped, and under I went. His hoof caught me just above the right eye and gave me quite the gash. I had to have 9 stitches. Still have the scar.
On a weird note...my husband and daughter also had to have stitches above their eye at one point, so we all match!
And OUCH, with a hatchet? OOOOOOOOH, that had to hurt!

Way back in the day, a friend had a cheap playset - You know, swings, a slide, ad nauseum - the kind that was made of steel, but had metal screws sticking out all over the place. I remember swinging around on the playset's metal monkey bars, only to swing back onto a screw poking out of one of the playset's posts and letting go as soon as the pain hit.
I ripped the back of my leg open, quite viciously. Thirty years later I still have a scar from it. When my folks took me to the hospital, I recieved my first stitches. Honestly, the shots I had to get (we're talking about 1976 or so here) at the time seemed to hurt far worse then the actual injury.
Anyway, that's my little story. My leg was sewed up tight. The overall damage wasn't that severe in the long run (though it was awfully bloody at the time), but it was an incident of which I could never quite let go.

When I was little, my sisters and I were playing house out in the back yard. One of my sisters was the mommy and she put us to bed on a blanket on the ground. Then she went to work in the "garden"...with a real garden hoe. She was chopping the ground near where my sister and I were "sleeping" and chopped into the top of my head with the hoe! 35 stitches! I still hear the sound of that hoe hitting my head.

Appendicitis, the stitches were internal and dissolving. Nothing outside except for some heavy duty tape.

A car accident. I was hit by a drunk driver who ran a red light and was being chased by the cops who were following at a distance without sirens or lights on in hopes of calming and slowing the driver down.

Do staples count?
I had to have brain surgery and they stapled me back together instead of stitching.
I did have stitches when I had a muscle biopsy done
