Visit the local U.S. Consulate Office in that country, and they'll get you a replacement.

Contact your countries nearest embassy/consulate.
It is a lot easier if you have the copy of your orginal passport, but can be done with out.
The embassy will help you sort out all that you need, including new visas (some countries such as China, you need a visa to stay any length of time, you can not leave with out one)

Contact the closest US or Canadian embassy in the country you are in. They will help. - assuming you are American.

If in an emergency other embassies can help too. Find one of a country that speaks the same language you do.

You should also carry black and white photo copies of your passport somewhere in your luggage - in case you lose the real thing. It will be easier to have a new one issued.

Depending on your nationality, you should head for your embassy or high commission or consulate or trade representative office (whichever is present) and seek a temporary replacement to allow you to get home first. Then you get a replacement for your lost passport.
