That depends. Is your last name Jones? I think it would be Nurse Dr. Jones (or whatever your last name is), or you could always go with Nurse Jones, PhD. In the hospital, I would hope you would just stick with Nurse Jones so you didn't confuse any of the patients.

If you were a nurse and wanted to get the highest education as possible, you would get your masters in nursing, and your title would be Nurse Practitioner Jones.

Anyone who has a PhD can use Dr. in front of their name (you do have a doctorate degree). More likely, you would have your name and your credentials behind your name including PhD.
