The customs form is published by the country you will be entering. If you are going into Canada it's an E311, available at http://www.cbsa-asfc.gc.ca/E/pbg/cf/e311/e311-06e.pdf . If you're travelling to the United States, it's a 6059B, depicted at http://www.customs.gov/xp/cgov/travel/vacation/sample_declaration_form.xml but they want you to get an original at a port of entry..

Maybe a travel agent. You need to fill them out on a cruise ship too.
