Not "anytime". It has to be arranged ahead of time. There are some leaders that aren't going to be welcomed at the White House. When a visiting head of state comes to Washington, it's usually a very big ceremony.

Some of the time but always if it's a State or official visit.

Sometimes another President or leader pops in for an unofficial visit or business with the UN or some company...then not necessarily.

The President of Mexico was in town recently and GWB didn't meet with him.

If the foreign President is a two-bit dictator like Fidel Castro who comes to the UN once in a while, he's pretty much ignored.

If it is an official STATE VISIT yes. For "working visits" there is only limited protocol.
