It needs cleaned. On the back you can see a border around the rubber ball. You can see two arrows indicating the direction in which it turns. Once the border is loose, the ball can be removed. See all the crud? Take a cloth and with alcohol or, I use witch hazel, clean the inside and the ball. When it is thoroughly dry, replace it. spin the ball to test.

Clean the ball.

Change the mouse pad

If its an optical mouse, put it on a piece of paper, if its on wood the beam loses its sight.

Change the speed settings in the control panel

Buy a new mouse

I suspect you are not alone with this problem. My mouse has this "disease" too and it is very annoying by times. Nine chances out of ten it is because that little ball needs to come out and the sides that holds the ball in needs cleaned periodically. It's amazing how often this gizmo needs cleaned and how dirty it can get!!

You can try cleaning the ball on the bottom of the mouse w/ a cotton ball soaked in rubbing alcohol. If that doesn't work, replace your mouse, you can get them at a cheap price at Wal-Mart
